Client seemed to be in a relaxed mood, as evidenced by his soft tone and laid back energy. Client displayed appropriate language while talking with his peers. Client participated in individual therapy without getting frustrated or upset. Client communicated the positive reinforcement that was provided by TDT staff. Client exhibited good listening skills, waiting his turn to speak, and giving his undivided attention. Client was able to complete his classwork before going to his next break. Client showed excellent manners by saying please, yes ma'am, and thank you. Client exhibited low impulses and remaining in quiet area while he studied for class. Client did a good job reducing outbursts and being courteous of others. Client was monitored and supported with following the rules in session, class. Client was in a cheerful mood, as evidenced by his content and happy voice. Client communicated he felt refreshed after taking a break. Client utilized acquired skills learned in teletherapy to focus on his work and make good choices. Client appeared focused and undistracted by others who were off task. Client transitioned smoothly to class following morning meeting. Client was able to obtain ISP objectives 1 2, and 3, as evidenced by displaying self-control and respect for peers, teachers, and mental health professional. (PM) QMHP will conduct a morning session to debreif and talk about plans for the day and how he plans to excel in each class. QMHP will give client a break in the morning and evening. QMHP will encourage client to stay focused in class and ignore distractions. QMHP will help client develop healthy self-esteem by reducing level of anxiety and emotional reactions related to school and other independent functioning. QMHP will educate client about being respectful, encouraging, mindful, and fair to peers and authority while making good decisions in school. QMHP will collaborate with parent regarding behaviors and performance in school. QMHP will review and give feedback regarding ADHD and its causes and affect toward client well being and academic functioning. QMHP praised client for being on task and following directions. QMHP educated client about appropriate behaviors. QMHP processed client thoughts and feelings. QMHP engaged client in conversation regarding his recent behaviors, progressions, and setbacks. QMHP explored client’s responses and provided appropriate feedback. QMHP supported client making good decisions and having positive behaviors in school. QMHP supported client complying with classroom rules and expectations. QMHP talked to client about his confidence levels and how they can affect his thoughts and behaviors in school. QMHP encouraged client to utilize positive self-talk as a means of decreasing anxiety and managing frustration when encountering difficult or challenging circumstances. QMHP reminded client of his strengths and abilities. QMHP praised client for his efforts and being respectful to his teachers and classmates. QMHP reviewed ISP goals and objectives. Client stated he loves playing outside after school. Client particpated in reviewing key concepts. Client showed good understanding of what it means to be respectful, encouraging, mindful, and fair to others. Client is observed correcting his mistakes and making good choices. Client was able to utilize coping skills to eliminate stressors and unwanted feelings. Client displayed self determination to stay focused and pay attention in class. Client utilized meditation and relaxation techniques. Client was seen decreasing anger and frustration. Client reported a decrease in symptoms following the relaxation techniques. Client expressed positive thoughts and feelings about himself and others. Client seem to be starting off the week on a good note, as reported by his teacher. Client smiled and said thank you. Client discussed mental illness and its causes as a group. Client displayed interest in learning about ADHD. Client believes he will get better grades in school by staying in class and listening to his teacher. Client was acceptive of support and encouragement. Client exhibited good virtual behaviors. Client is supported building healthy relationship with his counselor. Client displayed positive self talk, ability to follow instructions, role play of appropriate way to act in school, increased communication and exhbiting the positive reinforcers that were provided by QMHP
